Qualifying for assistance through the Xterra Health Foundation begins with a simple, supportive process created to honor your service and help you move forward with confidence. Our mission is to ensure that cost never stands between a veteran and the medical documentation needed to pursue a fair VA disability claim. FAQs

Who can apply for help through the Xterra Health Foundation?

Any U.S. veteran or surviving spouse who needs medical evidence for a VA disability claim may apply. Our priority is veterans who cannot reasonably afford a private Nexus Letter or DBQ on their own.

The Foundation can help cover part or all of the cost of:

  • Nexus Letters
  • DBQs (Disability Benefits Questionnaires)
  • In some cases, follow-up medical-legal review directly related to the Nexus or DBQ

All services are provided by independent licensed medical providers familiar with VA disability claims.

We use the Federal Poverty Guidelines (FPG) and your household size to determine eligibility. The table on this page outlines the income ranges for each tier:

Free Tier (100% FPG) – Foundation covers the full approved service cost$200 Tier (up to 170% FPG) – Reduced flat fee$300 Tier (up to 255% FPG) – Further reduced flat fee

For households larger than 8, use the “per additional person” amounts listed below the table.

If your income is above 255% FPG, you may still work with Xterra Health at standard pricing, but you would not qualify for Foundation assistance.

After your initial interview, we will request:

  • Your most recent 30 days of bank statements (required)
  • Most recent tax return (if available)
  • Recent pay stubs
  • Social Security, pension, or VA award letters
  • Any additional documentation that helps confirm household income

This information is used solely to verify your eligibility tier.

Start by completing our short online intake form. Once we receive your intake, a team member will contact you for a brief initial interview. No documents are required before this call. After the interview, we will provide instructions on where and how to submit your income documents and any necessary medical records. Once everything is submitted, we will review your information and confirm your eligibility tier.

No. Approval only means the Foundation can assist with cost if the provider determines a Nexus or DBQ is medically appropriate. The final decision to write a medical opinion is made by the independent provider after reviewing your records.

Most applications are reviewed within a few business days after all documents and records are submitted. High volume or missing information may cause delays, but we will keep you updated throughout the process.

No. The Foundation only assists with the cost of obtaining medical evidence. We do not communicate with the VA on your behalf, change your benefits, or influence claim decisions. You are responsible for submitting any Nexus Letters or DBQs to the VA.

Yes. If your financial situation changes or you have a new claim requiring medical evidence, you may reapply. Updated income information may be required.

You can still complete the intake form. During your initial interview, our team will explain which records are needed and how to submit them once your eligibility is confirmed.
